Step 2. Confirm your appointment time

Once your ‘First Appointment Form’ is complete, a member of the Philosophy Team will contact you within 24 hours to select or confirm your appointment day and time.

Step 3. Please print and complete the following forms

Before attending your first appointment with your dietitian please complete:
Three-day food record (printer friendly version, 364kb)
Activity record (printer friendly version, 211kb)

Bring your completed food and activity record to your first appointment.
